Scholarships

There are a limited number of travel scholarships available, and they will be awarded based on the following criteria:

Evidence of prior interest and experience in addressing and eliminating health disparities

Benefits of attending the Summit for the candidate and the community he/she serves
Financial need

Please note that scholarships will be awarded on a rolling basis, and the final deadline for receiving applications is Friday, October 31. Applicants should complete the attached form and submit their request to summitmaterials@ncmhd.nih.gov with the subject matter titled "Travel Scholarship Request" In addition, only U.S. citizens or non-citizens with permanent residency status are eligible to apply.

There is a limited number of scholarships and selections will be made by October 31, 2008.

The scholarship submission period has ended.
