Upon confirmation of receipt of your completed registration form, a guestroom will be reserved in your name at the Hyatt Regency Crown Center Hotel. A credit card to guarantee all reservations and to cover the costs for unauthorized cancellations or no-shows is required to make your reservation. In addition, your credit card is required at check-in to cover incidental room charges such as room service, phone calls, pay-per-view movies, etc. Your credit card will be charged only for unauthorized cancellations, no-shows, and incidental room charges. If you do not have a credit card to cover incidental room charges at check-in, a cash deposit of $50.00 per night will be required. This amount is fully refundable if no incidental room charges are incurred. For non-sponsored participants, specific lodging costs are listed below for your convenience.

Please do not contact the hotel directly to make your reservation. All lodging arrangements must be made through B L Seamon Corporation (BLS).

You should plan to arrive by 12:00 p.m. on Thursday, June 18, 2009, and plan to depart after 7:00 p.m. on Saturday, June 20, 2009, the last day of the conference, unless otherwise authorized. If you are a Youth Initiative Ambassador, you should plan to arrive no later than the evening of Wednesday, June 17, 2009.

Lodging for approved participants will be paid by the NHSC for authorized nights only. For non-sponsored participants and those sponsored participants who wish to stay on their own, the lodging rate at the Hyatt Regency Crown Center is $124.61 ($107 + 14.85% tax + $1.72) per night for a single-occupancy room.

LODGING CANCELLATION POLICY
If you need to cancel your reservation, please contact Sidnitra Bates at B L Seamon Corporation immediately to avoid a direct credit card charge of 1 night's lodging. Cancellations after Tuesday, June 16, should be made directly with the hotel. If you cancel your reservation, obtain a cancellation number for your records. (You must provide a cancellation number for any disputed charges.)

All guests who cancel reservations 24 hours or less prior to the scheduled arrival date will incur a direct credit card charge of 1 night's lodging. Any fees associated with unauthorized cancellations will be at the participant's expense.
