Public Comment

Thank you for your interest to provide public comment at the December 2013 webinar of the Chronic Fatigue Syndrome Advisory Committee.

Members of the public will have the opportunity to provide public comment during the webinar via telephone, pre–recorded video or written comments only. Please refer to the following information regarding providing public comment:

Pre–registration for the webinar is required in order to submit public comment. An individual who would like to present comments should note this when completing the registration form. The deadline to register and submit public comment is Friday, November 29, 2013. Please refer to the agenda for scheduled public comment periods.

Each speaker via telephone or pre–recorded video will be limited to five minutes per speaker. We will give priority to individuals who have not provided public comment within the previous year.

Via Telephone

  • You will be notified by email of your assigned date and time to give presentation no later than Wednesday, December 4, 2013.
  • The CFSAC Committee will contact you during your assigned time frame at the phone number provided in this registration from. Please prepare to be available during the entire time frame assigned.
  • We regret that international calls cannot be accommodated.

Via Pre–Recorded Video

  • Pre–recorded video comments are an acceptable form of public comment.
  • Only pre–recorded videos uploaded to YouTube (http://www.youtube.com/upload) will be accepted and played during the public comment period if received by Friday, November 29, 2013.
  • Videos that exceed 5 minutes will not be accepted.
  • Individuals wishing to submit a video for public record should include the YouTube URL address with their registration form or send an electronic copy of their testimony in advance to: CFSACDec2013@seamoncorporation.com no later than Friday, November 29, 2013.

Via Written Comment

  • You are no longer required to submit a written copy of your testimony as in past years unless you wish to have it included in the public record.
  • Individuals wishing to submit written comment for public record should include it with their registration form or send an electronic copy of their testimony in advance to: CFSACDec2013@seamoncorporation.com no later than Friday, November 29, 2013.
  • The document for public record must not exceed 5 single–spaced, typed pages, using a 12–point typeface; it is preferred that the document be prepared in the MS Word format.
  • Please note that PDF files, hand–written notes, charts, and photographs will not be posted on the CFSAC Website. Individuals who wish to view this material after the webinar should send an email to CFSACDec2013@seamoncorporation.com.

Requests to participate in the public comment and provide testimony will not be accepted through the CFSAC mailbox. Also, the CFSAC mailbox will not respond to questions about specific public comment requests. These requests and/or inquiries should be directed to CFSACDec2013@seamoncorporation.com.

Only testimony submitted for public record and received by Friday, November 29, 2013 will be part of the official record and will be posted to the CFSAC website at http://www.hhs.gov/advcomcfs. Materials submitted should not include sensitive personal information, such as social security number, birthdates, driver's license number, state identification or foreign country equivalent, passport number, financial account number, or credit or debit card number. If you wish to remain anonymous the document must specify this.

You will receive an email by December 4th confirming the date and time for your public comment.
